Not exactly what you're looking for but you could use eesel.app to find pages instead of the address bar. You can configure a deny list for specific pages on eesel.app. The Firefox add on is new - https://addons.mozilla.org/en-GB/firefox/addon/eesel/. Source: over 2 years ago

Fax resources can help apply to assistance programs or financial institutions from anywhere. A fax is considered equal to sending a physical document by physical mail under U.S. Federal law. Websites useful for sending faxes are faxzero.com and gotfreefax.com. There are limits to how many faxes are sent a day. Fax Zero allows up to five faxes at three pages per day. It's pretty broad - pretty much anything bought off a website. Recent examples for me include Tire Rack, Troy-Bilt and Kiehls (coincidentally fradulent charges that were of course reversed), gotfreefax.com, thredup, costco via the website, ebay etc. Source: about 2 years ago
Call your rehab and ask if you can email them a copy of the form or if affiliated with a health system with EPIC, maybe you can upload if they offer a "mychart" website - otherwise there are free online fax services - I've been using http://gotfreefax.com for years - sends you a confirmation when completed. Because years have elapsed, departments & personnel change & so much work is remote now - workers check their work voicemail from home. You may need to complete as much of each ECF as possible then scan & email to each prior employer who scans & emails it back.
