Given that this is an online platform and that they have to optimize the analysis, many scans will be done quickly, or "messed up", which means that an anti-virus on virustotal could not detect anything, whereas an anti-virus on a private computer would. You can still get it done without insurance, it's just really expensive. It will be thousands of dollars cheaper to get insurance for a year or so and have the surgery covered than to try and pay for it out of pocket, so it's worth maybe getting insurance.
